Question #1 RFQ #2012-0001 Transportation Services:Do the RFP’s really need to be typed in since they are written in Word and not in PDF format so we can type in the information?Answer

The RFP document will only allow you to type in your response directly on Exhibit C – Request Response Submittal Coversheet, page 16. After typing your response directly on Exhibit C, agencies must print it out, and sign the coversheet. It is up to the agencies whether they choose to type the remaining documents requested on page 3 of the RFP. All documents noted on page 3 must be included in your proposal.

Question #2 RFQ #2012-0001 Transportation Services:

What is a type 2 ADA complaint vehicle?


Type 2 ADA Vehicles are generally bus type vehicles with walk-up hand rails and a lift for wheelchair access. They are not van type vehicles.

However, in general, the vehicles must comply with the American Disabilities Act for passengers.

Question #3 RFQ #2012-0001 Transportation Services:

Does the price sheet have to be typed or can it be in handwritten?


The RFP did not specify that proposals must be typed. However, it is the standard in the industry.

Question #4 RFQ #2012-0001 Transportation Services:

What is PVC and VDDP inspections or where you implying PUC?


There is a typo in this question, PVC should be PUC. PUC stands for Public Utilities Commission and VDDP stands for Vehicle for Developmentally Disabled Persons. PUC provides authorization for organizations to operate as a Private Carrier of Passengers. VDDP is a DMV drivers certificate, which authorizes organizations to operate vehicles transporting disabled persons.

Question #5 RFQ #2012-0001 Transportation Services:

Do you need proof of inspections for CHP –as proposed to this RFP.


Yes, organizations must show proof of CHP inspections.


Question #1I have a couple of concerns with the contract.  Very minor.  I have them documented in the submittal form.  Do I need to discuss those questions with you beforehand?Answer to Question #1

No, your contract concerns do not need to be communicated to me beforehand. Please feel free to document them in the submittal form.

Question #2

The required information detailed on page 3 of 19  is addressed in our Letter of Interest.  Is that sufficient?  We will provide all the information requested when we present and place a physician.

Answer to Question #2

As long as your submittal response answers all of the required information, this should suffice.

If I have a physician who is interested in providing LT coverage under this contract do I need to send it under the same cover or separate cover?

If this physician is still providing services under your agency, your agency’s submittal package should suffice.  If this physician is looking to provide services under his/her own license and insurance and as an individual contractor, then he/she must submit his/her own separate submittal package.

Question #3

Jackson & Coker already has an active Contract with Neighborhood House Association to provide locum tenens or temporary psychiatrists.

Therefore, do we need to respond to RFQ 2012-001 for Psychiatric Consultant Services?

Answer to Question #3

Yes, all agencies or individual contractors that would like to provide psychiatric services to the Neighborhood House Association Project Enable Mental Health Program must respond to the RFQ 2012-001.

Question #4

If so, is there a possibility the due date can be changed to 11/30/12 so that we have time to put together a decent proposal?  I just received notice of the proposal only two days ago.

Answer to Question #4

Since the RFQ has already been extended once already, at this time, we are not open to extending the due date again.  Please note that this is a request for qualifications not a request for proposals, which may be less time intensive. 

Question #5

Many of the people who do our proposals are out of the office this week and won’t be returning until Monday, 11/26 which is the revised due date for this RFQ.

If it is not an option to change the bid due date to 11/30/12, then can we possibly submit a proposal via email by close of business on Monday, 11/26/12?

Answer to Question #5

Yes, submittal by E-mail, in PDF format, will be accepted provided the signed original NHA Request Response Submittal Cover Sheet is received within twenty-four (24) hours after the submittal deadline.
