Safe Connections

Safe Connections is designed to meet the mental health needs of clients who have had repeated use of emergency psychiatric services. Our goal is to reduce the stigma & reduce hospitalizations by providing individualized support through short-term intensive case management, therapy, and linkages to treatment and community services such as housing, transportation, peer support, life skills and medication support.

Eligibility

  • Age 18 years and older 
  • Referred by Sharp Grossmont, Bayview, or Paradise Valley Hospital 

Payment

Safe Connections accepts Medi-Cal insurance. Those who are not insured are charged an annual fee based on their ability to pay.
