Neighborhood House Association's Case Management and Peer Support Services program helps clients maintain and improve health wherever possible and gain access to resources and services that meet each client's individual level of need. The program provides comprehensive, ongoing assistance to individuals with HIV/AIDS. Case Management staff advocates on behalf of clients to ensure proper treatment and care. The program consists of three components: intensive case management for ex-offenders, general case management and peer advocacy.

Transportation Services
Neighborhood House Association's Transportation Services provides door-to-door assisted and unassisted transportation to low-income residents of San Diego County who are affected by HIV/AIDS. Unassisted services refer to bus passes and coaster passes that are provided to clients via case management programs.

Case Management helps clients access the following:
  • Appropriate medical, mental health and dental care as needed
  • Assistance with HIV/AIDS medication costs
  • Social security referrals for eligible clients
  • Applications for low-income housing
  • EARP (Emergency Assistance Resource Pool) assistance for housing, utility and clothing costs
  • Assistance with obtaining food or meal delivery
  • Assistance with transportation to and from medical appointments
  • Referrals to treatment facilities for in-patient and out-patient substance abuse treatment
  • Referrals for free or low-cost legal services
  • Referrals for support groups
  • Referrals for vocational training
Transportation for qualified individuals can only be provided for the following purposes:
  • Medical care
  • Oral/dental care
  • Pharmacy services, if no delivery option is available
  • Mental health services
  • Substance abuse services provided in an out-patient setting
  • Case management, including visits to client's case manager or peer advocate
